Insurance Related Forms

Units asking for a Certificate of  Insurance need to fill out the request form and send to the council service center. For a fundraising activity, you must submit your unit money earning application as well.

If the requester must have specific wording, send their requirements with the request.  Be aware the any such request must be reviewed by National BSA and their insurance carrier, and may be rejected, so encourage the requesting party to accept the standard Certificate of Insurance.

Please allow 2 weeks to process requests for Certificates of Insurance.
